(1) How does the
debit card work?
It works like a MasterCard® card, with the value of
the participant’s account(s) contribution stored on it.
When you and/or your dependents have qualified eligible
expenses at a business that accepts MasterCard debit
cards, simply use your Benny® Prepaid Benefits Card.
The amount of the qualified purchases will be
automatically deducted from your account and the pre-tax
dollars will be electronically transferred to the
provider/merchant for immediate payment.

| (2) How many cards
will I receive and can I order additional cards? |
| You will receive two Benny® Prepaid
Benefits Cards (unless you have ONLY a Qualified
Transportation Account, in which case one Card will be
issued). If you would like additional Cards, you can
order them by completing and submitting a
Debit Card Request Form via fax to (800)818-0829.
There is a fee of $10 per each additional card ordered,
which will be deducted directly from your account. At
least 48 hours prior to first usage, you must activate
your card. The easiest way to do this is via our
website,
www.tri-starsystems.com. Each cardholder should
sign the Card with his or her own name.

What dollar amount is on the Benny® Prepaid Benefits
Card when it is activated?
For Health Care FSAs, the
dollar value on the Card will be the annual amount that
you elected to contribute to your account during your
benefits enrollment. It’s from the total dollar amount
that eligible expenses will be deducted as participants
use their Cards or submit manual claims. Some other
types of accounts, like Dependent Care FSAs, HRAs, and
transportation account, are funded incrementally at each
pay period, so it is especially important to be aware of
account balances in order to avoid Card declines at the
point of service.

can I check the balance I have on my card?
You can check your balances
by logging into your account
here. You will need your Social Security Number or
Account Number and Password. Your Account Number and
Password were sent to you in the Welcome Letter when you
first enrolled

(5) What type
of expenses can I use the card for?
Swipe the card for: health
and Prescription plan co-payments, Dental expenses,
Vision services and eyeglasses, and Over-the-counter
drugs. If prompted, you should select “Credit”.

What are some reasons that a Benny® Prepaid Benefits
Card might not work at point of sale?
The most common reasons why
a Card may be declined at the point of sale are:
o
The Card has not been activated or has
been used before the 48-hour period after activation is
over.
o
You have insufficient funds in your
account to cover the expense.
o
Non-qualified expenses have been included
at the point-of-sale. (Retry the transaction with the
qualified expense only.)
o
The merchant is encountering problems
(e.g. coding or swipe box issues).

What if I have an expense that is more than the amount
in my account?
By checking your account
balance often, you will have a good idea of how much is
available. When incurring an expense that is greater
than the amount remaining in your account, you may be
able to split the cost at the register (check with the
merchant). Alternatively, you may pay by other means
and submit the qualified transaction manually via a
Claim Form with the appropriate documentation to the
fax number/address on the form.

(8) Do
I have to send in receipts when I use my Debit Card?
You should always save
itemized receipts for purchases made with the Benny®
Prepaid Benefits Card, as you may be asked to submit
receipts to verify that the expense(s) comply with IRS
guidelines. Each receipt must show: the merchant or
provider name, the service received or item purchased
and the date and amount of the purchase.

(9) How
will I know if I need to submit my receipt(s)?
You will receive a request
for receipts via e-mail from:
BenefitCentralCardManagement@bennycentral.com. You must
fax a copy of the email
with required documentation to (800) 315-0737 within 15 days from the
request date.
VERY IMPORTANT NOTE:
You should make sure that you have a contact set up in
your email address book for the above email address and
also make sure that your email client or email server is
not blocking that email address. If you block the email
and do not receive it your card will be suspended after
the allotted time because follow up receipts were not
received. The card will remain suspended until the
outstanding receipts are received and approved or the
amounts in question are repaid..

What happens if I do not submit the requested receipt(s)?
If receipts are not
submitted as required, then the Card may be suspended
until receipts are received. You may be required to
repay the amount charged. You will be advised that the
Card has been suspended if a receipt is not received.
Submitting a receipt or repaying the amount in question
will allow the Card to become active again.

(11) What if
the Benny® Prepaid Benefits Card is lost or stolen?
Call Tri-Star Systems at
(800) 727-0182, Option 1 to report a Card lost or stolen
as soon as you realize it is missing, so we can turn off
your current Card(s) and issue replacement Card(s). If
the notification is after 2 business days, you may be
responsible for some of the charges. Replacement cards
are $10 each, which will be deducted directly from your
account.
